目的探讨肺癌患者外周血中白细胞及其亚类与肺癌肿瘤标志物糖抗原125(Ca125)、细胞角蛋白19片断(Cy FRA21-1)、铁蛋白(Fer)之间的关系。方法收集134例进行肺癌标志物检查的人群血常规资料,按照Ca125、CYFRA21-1和FER质量浓度上限将患者分为高浓度组、正常组;另外再收集已经确诊为肺癌的患者外周血,健康者外周血作为对照,ELSIA检测肿瘤患者和正常人血清中IL-8和TNF-α的质量浓度;密度梯度离心分选外周血中性粒细胞和淋巴细胞,q PCR检测IL-8和TNF-α的表达。结果相对于正常组,高浓度组中白细胞总数以及中性粒细胞计数显著升高(P<0.05)、淋巴细胞计数显著降低(P<0.05),中性粒细胞-淋巴细胞比率显著升高(P<0.05)。肺癌患者血清TNF-α水平和正常组相比无明显差异(P>0.05),而肺癌患者血清IL-8水平显著高于正常组(P<0.05)。肺癌患者中性粒细胞中TNF-α、IL-8 mRNA表达显著高于正常组(P<0.05);而淋巴细胞中TNF-αmRNA的表达比健康者显著增加(P<0.05),IL-8的表达水平和健康者比较无显著差异(P>0.05)。结论白细胞计数及其亚类与肺癌肿瘤标志物Ca125、CYFRA和FER质量浓度具有相关性。中性粒细胞和淋巴细胞之间的代偿机制可为肺癌的诊断提供依据。
Abstract:Lung cancer is one of the leading cause of death malignancy worldwide. The aim of this retrospective study was to assess the clinical value of leukocyte and leukocyte subset counts in lung cancer patients by investigating their relationship with tumor markers Ca125, CYFRA21-1, FER. According to the inclusion/exclusion criteria, total of 134 subjects were selected from outpatient of Renmin Hospital, Wuhan University from2014 to 2015. The serum levels of three tumor markers were measured using commercially available immunoassays;the serum levels of TNF-α and IL-8 were measured by ELSIA, and q PCR was used for quantification of TNF-α and IL-8 mRNA levels in neutrophils and lymphocyte. Data showed that leukocyte and neutrophil counts were increased with the elevation of Ca125, CYFRA21-1, FER concentration(P<0.05), while lymphocyte counts showed the opposite trend. Serum TNF-α level demonstrated no different between lung patients and normal controls.Neutrophil TNF-α mRNA level in patients were higher than that of normal control, but lymphocyte TNF-α mRNA level was lower in patients. However, serum IL-8 level in lung patients were much higher than that in healthy controls. IL-8 mRNA level demonstrated no different between patients and healthy controls. In conclusion, leukocyte and leukocyte subset counts are correlate with lung cancer markers. The neutrophil-lymphocyte interaction reveals a potential compensatory mechanism in lung cancer
